Print the form, fill it in and send it to Administration Building, Western Harbour Crossing, Yaumatei, Kowloon, Hong Kong together with a crossed cheque made payable to "Western Harbour Tunnel Company Limited" or "WHTCL". Post-dated cheque will not be accepted.
